The credentials may be submitted in one of these ways:

2- Fax the credentials to 317-237-5705 (please confirm delivery and receipt of the fax transmission).

3- Mail a copy of the credentials to (please notify the Education Director that these are being mailed):

***Sometimes the cards given by the safety organizations can take some time to get delivered to participants, for that reason, we can accept other proof of completion of the safety course.  The activity report that the instructor fills out can be copied and submitted.  Also, a letter from the instructor can be submitted.  If one of these is submitted for proof, please be sure the following information is included: name of course, name of participant, date of course, expiration date of certification, and instructor contact information.
